A new study suggests that heartburn may be a marker of the severity of heart failure. This is because in the heart, acid helps the heart maintain its natural balance and may result in less damage to cells.
Researchers at Harvard Medical School and Boston University Medical School reported that over-the-counter medications like Zantac, Nexium and Pepcid AC (esomeprazole) are all used to treat acid reflux (heartburn) in people who have heart disease or who take certain heartburn medications.
Some of those drugs were prescribed by doctors, while others were sold as dietary supplements that are taken by mouth.
While some people can't swallow pills, most can swallow small, over-the-counter medications. The researchers said that this suggests that the drugs may be used in patients who have heartburn, a common condition in which the heartburn is painful and lasts for long periods of time.
"We are examining whether the medication we have and the way we use it can provide similar benefits to those we have in the past," said, associate professor of medicine at Harvard Medical School and coauthor of the study.
Zantac and Nexium, which are used to treat acid reflux, are the most popular prescription medications. However, they are also prescribed off-label to treat chronic heartburn (also known as GERD).
The drug Zantac was developed by AstraZeneca and has been used to treat acid reflux since the 1970s. It was first approved by the FDA in 1981. The FDA approved Zantac in 1998.
A similar drug, Pepcid AC, is also prescribed to treat acid reflux. It was developed by AstraZeneca and was originally used to treat the symptoms of heartburn. It was approved in 2001.
A third of all heartburn sufferers have to suffer from heartburn because the medication can cause a painful, prolonged and sometimes painful period. Heartburn can cause serious health problems such as heart failure, kidney disease and, in some cases, death.
The study looked at 22 people, all of whom had heartburn for the first time, with an average age of 58 years old. All of the patients had to have a prescription for an acid-suppressing medication (zidovudine, lansoprazole, omeprazole) within the past six months.
Researchers found that over-the-counter drugs like Zantac, Nexium and Pepcid AC had fewer symptoms than prescriptions purchased from pharmacies. Over-the-counter medications were also less effective than prescription drugs.
"There is a lot of evidence that is positive about the role of these medications in the treatment of heartburn," said Dr. David S. Krumholz, professor of medicine at Boston University.
"There are several factors that could lead to more negative events, including the presence of other medical conditions," he said.
For example, the drugs might increase the risk of cancer in the stomach, which is the area of the upper stomach. Researchers also have a hard time quantifying that risk. They also need to consider whether the patients who take the drugs have the same risk for serious side effects as those who take the pills.
"A lot of the medications we have used for heartburn, we think they are probably safe in patients with heartburn," said S. D. Krumholz.
"We don't have a lot of data on how these medications affect the patients who are taking them. We have only one study done on the effects of these medications on people who have heartburn," he said.
Zantac, Nexium and Pepcid AC are the most popular prescription medications used to treat acid reflux. They are all taken by mouth and are taken twice a day. The drug has a long half-life in the body.
The study is published in the journal Gastroenterology.
